==== Two states of persistent pain ==== After persistent peripheral tissue damage, there is a release of several factors from the injured tissue as well as in the spinal dorsal horn. These factors increase the responsiveness of the dorsal horn pain-projection neurons to ensuing stimuli, termed "spinal sensitization", thus amplifying the pain impulse to the brain. Release of glutamate, substance P, and calcitonin gene-related peptide (CGRP) mediates NMDAR activation (originally silent because it is plugged by Mg2+), thus aiding in depolarization of the postsynaptic pain-transmitting neurons (PTN). In addition, activation of IP3 signaling and MAPKs (mitogen-activated protein kinases) such as ERK and JNK, bring about an increase in the synthesis of inflammatory factors that alter glutamate transporter function. ERK also further activates AMPARs and NMDARs in neurons. Nociception is further sensitized by the association of ATP and substance P with their respective receptors (P2X3) and neurokinin 1 receptor (NK1R), as well as activation of metabotropic glutamate receptors and release of BDNF. Persistent presence of glutamate in the synapse eventually results in dysregulation of GLT1 and GLAST, crucial transporters of glutamate into astrocytes. Ongoing excitation can also induce ERK and JNK activation, resulting in release of several inflammatory factors. As noxious pain is sustained, spinal sensitization creates transcriptional changes in the neurons of the dorsal horn that lead to altered function for extended periods.

=== Drug and weapons operations === Panama's and Noriega's involvement in drug-trafficking grew considerably over the early 1980s, peaking in 1984. Intensifying conflicts in Colombia, El Salvador, Guatemala, and Nicaragua had led to the creation of covert transportation networks that Noriega used to transport drugs to the U.S., particularly cocaine. During this period, Colombia's Medellín Cartel was also seeking allies. Noriega became intimately involved with their drug trafficking and money-laundering operations, and received considerable sums as protection money, bribes, or shares of profits. In June 1986, investigative journalist Seymour Hersh recorded a U.S. White House official as saying that reducing Noriega's activities could greatly reduce international drug trafficking. Hersh reported unnamed U.S. officials as saying that Noriega had amassed a personal fortune in European banks as a result of his illegal activities, as well as owning two homes in Panama and one in France. The wealth generated for the Panamanian military from drug-smuggling also helped stabilize the authoritarian government that it dominated. However, the military's control over wealth from illicit trade alienated the Panamanian business elite that had previously also benefited from such trade. Under Noriega, these profits were shared within the military less evenly than under Torrijos, eventually creating friction in the military leadership. == History == Golodirsen was developed by collaborative research led by Prof. Steve Wilton and Prof. Sue Fletcher in the Perron Institute and licensed to Sarepta Therapeutics by the University of Western Australia. In the clinical trial of golodirsen, dystrophin levels increased, on average, from 0.10% of normal at baseline to 1.02% of normal after 48 weeks of treatment with the drug or longer. The change was a surrogate endpoint and the trial did not establish clinical benefit of the drug, including changes to the subject's motor function. The pharmacological assessment of golodirsen did not include special population groups, e.g., pregnant and lactating women, the elderly, and people with concurrent disease states. As DMD predominantly affects male children and young adults, and golodirsen is indicated for the treatment of children, but primarily not for adult women, the elderly, and people with comorbidity, it was not evaluated on them. The US Food and Drug Administration (FDA) approved golodirsen in December 2019, under the accelerated approval pathway. The application for golodirsen was granted fast track, priority review, and orphan drug designations, and a rare pediatric disease priority review voucher.

RK2 was first isolated in connection with an outbreak of antibiotic-resistant Pseudomonas aeruginosa and Klebsiella aerogenes in Birmingham in 1969, as one of a family of plasmids implicated in transfer of ampicillin resistance between bacterial strains. Plasmids in the IncP-1 subgroup has been isolated from wastewater, agricultural soil, and hospitals. RK2 is approximately 60 kbp long and contains genes for replication, maintenance, conjugation and antibiotic resistance. The resistance genes confer resistance to the antibiotics kanamycin, ampicillin and tetracycline. In addition, RK2 contains a set of potentially lethal (to the cell) genes, called kil genes, and a set of complementary transcriptional repressor genes, called kor (short for "kil-override") genes, which inactivate the kil genes.
